Commemorative Opportunities

Legacies and memorials are important symbols of enduring communities. Memorial Healthcare Foundation and Memorial Hospital of Lafayette County recognize the importance of offering opportunities to leave signs of lasting significance for future generations. Donors to the Caring and Compassion…Close to Home Campaign are invited to participate in "naming opportunities."

Donors may choose to name a room, an area, or wing within the new complex. Giving at a significant level will provide donors with the opportunity to memorialize a family member, honor the lifelong community contributions of an entire family, or display the donors’ name as a sustaining commemorative. This provides the enduring value of history for those who will make use of the services provided at Memorial Hospital of Lafayette County in the days and years to come.

Donors may choose an area (or areas) with a naming value equal to or less than the amount of their gifts. Not all rooms or areas have been designated with a gift value. Should a donor wish to name one of the undesignated areas, of course, special arrangements can be made. It is not intended that the assigned value for each commemorative opportunity match the true cost of construction and equipping of any given area.

While some plaques will be located in the area of the commemoration, many others will be included more appropriately on an honor wall located within the Hospital.

Prospective donors who wish to discuss commemorative opportunities are invited to call Tammie Evenstad, Director of Community Relations, at (608) 776-4466.

Donor Recognition
Donors are at the heart and center of the efforts for Memorial Healthcare Foundation and Memorial Hospital of Lafayette County. Every donor’s gift is important. Every donor’s commitment is a sign and symbol of hope for the future of our community and its members. All gifts will be acknowledged with gratitude.

All donors to the campaign above $500, whether or not they are participants in a specific naming opportunity, will be listed and acknowledged in a special way within the facility. Donors will be contacted to ensure that their names appear within this recognition in the form they wish. Those donors who choose not to be so acknowledged will be included with gratitude as "Anonymous."

At various times it will be appropriate for Memorial Healthcare Foundation to acknowledge to the broader community through various forms of media the generosity of our donors to the campaign. No individual dollar amount will be identified with any particular donor although groupings of donors within categories of giving may be published. Publicly announcing gifts is important for several reasons, not the least of which are the acknowledgment of widespread individual support and the education of others about the importance of participation in common community efforts. We hope all of our donors will allow us to acknowledge them in this way; however, if anonymity is requested, that request will be honored.

This is the chart of all available commemorative opportunities. It is important to note that the reserved list is as of 9/12/03.
